Job summary

Primecare Home Care Services is a licensed private home care provider delivering skilled nursing, personal care, companionship and structured family caregiving in clients' homes. You will implement customized care plans, support daily living activities, provide compassionate care, take vitals, and communicate with the supervisor.

The role requires HHA/PCA certification, a valid driver's license, reliability, and a willingness to work in-home settings.

Qualifications

• Experience in home health care as a Personal Care Assistant
• HHA or PCA Certification required
• Valid Driver's License with reliable transportation
• TB Screening/PPD Chest X-Ray within 30 days
• BLS/First Aid & CPR in-person
• Proof of eligibility to work in the U.S. and ability to pass background check

Responsibilities

• Carry out tasks on clients and follow care plans
• Help clients maintain personal hygiene and activities of daily living
• Take and record vital signs
• Provide companionship to clients
