Job summary

PACE Southeast Michigan is seeking a Direct Home Care Worker to provide in-home and community-based support, following the participant's care plan and CMS guidelines.

Responsibilities include personal care, meal prep, mobility assistance, medication reminders, and transportation to appointments, working to maintain participant independence with sensitivity to diverse backgrounds and dementia care needs.

Qualifications

• Minimum High School Diploma or GED required.
• Must have minimum 2 years of relevant work experience.
• Must have completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI.
• Annual in-service training including infection control, TB testing, safety training, and BLS.

Responsibilities

• Personal care assistance with activities.
• Assistance with meal preparation and feeding.
• Assistance with toileting and incontinence management.
• Performing simple treatments as needed.
• Shopping and running errands.
• Medication reminders.
• Laundry and homemaking services.
• Extended hours care.
• Transportation to appointments and off-site support.
