Job summary

Affinity Health Management in Waterville, Maine seeks a Medical Social Worker (MSW) to implement standards of care for hospice social work services. You will support patients and families with psychosocial needs as a core member of the Interdisciplinary Group, guided by the plan of care and ongoing assessments.

The role requires CSWE-accredited MSW, hospice experience preferred, and strong communication/organizational skills.

Qualifications

• A CSWE-accredited Master’s degree in Social Work (MSW) is required.
• Minimum of one year of health care experience; hospice experience preferred.
• Experience in hospice care preferred.
• Demonstrates strong verbal and written communication and organizational skills.
• Offer contingent upon satisfactory references and criminal background checks.
• Prolonged walking/standing; good visual/hearing to perform social work duties.
• Must be a licensed driver with a vehicle and valid insurance.
• CPR certification required (hands-on verification).

Responsibilities

• Assesses psychosocial status of patients and families/caregivers and communicates findings to the team.
• Performs social evaluations and plans interventions; provides crisis intervention as needed.
• Assesses cultural needs and educates the interdisciplinary group accordingly.
• Educates patients and families on advanced directives and care planning.
• Participates in developing individualized care plans with patient and family.
• Assists team in understanding social/emotional factors related to health problems and death.
• Assists with funeral planning and related responsibilities for families.
• Completes and documents accurate clinical notes and records changes/outcomes.
• Communicates with physicians about patient needs and plan changes.
• Coordinates care with community health professionals and agencies.
• Provides information and referrals for patients and families.
• Acts as liaison between patients, families and community agencies.
• Maintains collaborative relationships to support patient care.
• Develops contracts with public/private agencies as resources.
• Fulfills mandatory reporting requirements within required timelines.
• Teaches coping techniques to patients/families and works with the interdisciplinary group.
• Maintains safe environment and continuity of care for patients.
• Works with family/caregiver and team to meet emotional needs.
• Attends interdisciplinary group meetings.
