• Must be a licensed driver with a vehicle and CPR certification.

Aufgaben

• Assess psychosocial status of patients and families/caregivers.
• Provide crisis intervention and counseling as needed.
• Educate interdisciplinary group on culture and advance directives.
• Develop and maintain individualized plan of care with patient/family.
• Attend interdisciplinary group meetings and coordinate with physicians.
• Maintain accurate clinical notes and report changes.
• Participate in on-call duties and quality improvement activities.
• Fulfill other duties as delegated.
