Patriotcares seeks a Hospice Social Worker to manage the social service needs of hospice patients and families. You will provide counseling, coordinate care with nurses and staff, and connect families to community resources to support comfort and dignity at end of life.

The role requires an MSW or BSW from a CSWE-accredited program and SC licensure (LBSW/LMSW/LISW-CP), with at least 1 year of healthcare social work experience; hospice experience is preferred.

Formación

• Requires MSW or BSW from CSWE-accredited program.
• Licensed as LBSW, LMSW, or LISW-CP by the State of SC.
• Minimum of 1 year social services experience in a healthcare setting; hospice experience preferred.

Responsabilidades

• Assess psychosocial needs within 72 hours of admission by interviewing patient and family.
• Develop and update the plan of care with the interdisciplinary team biweekly.
• Coordinate psychosocial care and connect families with community resources.
