Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care in patients' homes and community settings. You will assess, plan, and coordinate end-of-life care with families and the IDT to ensure high-quality, compassionate support.

Qualifications include CT nursing license and at least one year of nursing experience, with preference for BSN and home care exposure. Travel to Mansfield region communities is expected.

Qualifications

• Graduate of an Associate’s or Diploma in nursing program.
• One year nursing experience OR one year nursing experience in a specialty.
• Licensed to practice in the State of Connecticut.
• Preferred BSN from NLN-approved program.
• Med/surgical or home care experience is a plus.

Responsibilities

• Provide skilled nursing care to hospice clients in home or community settings.
• Conduct comprehensive nursing assessments based on referrals and patient status.
• Develop and adjust care plans with the patient, family, and interdisciplinary team.
• Coordinate care and communicate with physicians regarding patient needs.
• Educate patients and families on signs, symptoms, and care strategies.
• Supervise LPNs and Home Care Aides as part of the plan of care.
• Document care accurately in the patient record and ensure timely follow-up.
