Job summary

Alc-Health-Care-Services seeks a part-time PRN hospice RN to plan, coordinate, and deliver in-home hospice care, guiding patients and families in comfort-focused nursing. The role emphasizes community health education and collaboration with the hospice team.

The nurse will perform comprehensive assessments, develop care plans, and administer treatments in patients' homes, ensuring quality, compassionate care and compliance with state requirements.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years in acute care in institutional setting.
• 1–2 years in home care experience preferred.
• Current RN licensure in state and CPR certification.
• Bachelor's degree with hospice care experience preferred.

Responsibilities

• Conduct initial and ongoing patient assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with patient and family involvement.
• Reassess patient and family needs regularly and adjust plan accordingly.
• Administer medications and treatments per physician's plan.
• Counsel patient and family to meet nursing needs and self-care goals.
• Provide health instructions and support activities of daily living.
• Coordinate with physician and hospice team to address changes in condition.
