Job summary

Absolute Hospice, Inc. is seeking a Hospice RN to provide hands-on nursing care in patients' homes or residences, with PRN and weekend on-call hours available. The role emphasizes education for patients and families, comprehensive assessment, and collaborative care planning.

You will evaluate patient needs, coordinate treatment plans, document thoroughly, and adhere to professional standards while supporting discharge planning and coordination with staff and physicians.

Qualifications

• AS degree in Nursing required
• Minimum 2 years nursing experience including hospice or home health preferred
• Valid driver’s license and clean driving record
• Excellent writing, communication and charting skills
• Ability to lift/transfer patients as needed

Responsibilities

• Educate patients and caregivers on conditions and steps for recovery
• Continually evaluate patient needs
• Coordinate and revise treatment plans
• Render nursing care and treatments as ordered by physician
• Participate in Quality Assurance/Quality Improvement processes
• Prepare accurate, timely reports and documentation
• Adhere to practice standards for patient care
