Job summary

ALC Health Care Services is seeking a Registered Nurse for a PRN hospice role, traveling to patients’ homes. The RN will plan, organize, and direct hospice care with emphasis on community health education and home-based services.

Essential duties include comprehensive patient assessments, implementing the plan of care, coordinating with the interdisciplinary team, and counseling patients and families. Requires state licensure, CPR, and hospice experience preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, with one year of hospice care experience preferred.
• Management experience not required.

Responsibilities

• Performs initial, comprehensive and ongoing patient assessments to determine hospice needs.
• Delivers professional nursing care using the nursing process.
• Writes and initiates the plan of care; revises as patient needs change.
• Regularly re-evaluates patient and family needs and adjusts plans accordingly.
• Administers medications and treatments as prescribed.
• Counsels patients and families to meet nursing and related needs.
• Provides health care instructions based on assessment and plan.
• Assists patients with activities of daily living and promotes self-sufficiency.
