Job summary

Alc-Health-Care-Services is seeking a PRN Registered Nurse for hospice care, traveling to patients’ homes and communities. Pay is per visit for admissions, deaths, follow-up visits, and recertifications, reflecting a flexible PRN schedule.

The RN will assess, plan, and implement nursing care, collaborate with families and the hospice team, and provide education and support in a home-care setting.

Qualifications

• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Prolonged or considerable walking or standing; able to lift, position and/or transfer patients; valid driver’s license and insured vehicle.

Responsibilities

• Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using all elements of the nursing process.
• Assesses and plans care; develops and revises the plan of care with patient/family input.
• Re-evaluates patient and family needs; initiates revisions to the plan of care as status changes.
• Interacts with family/caregiver and hospice team to meet emotional needs; attends interdisciplinary group meetings.
