Job summary

ALC Health Care Services seeks an experienced hospice registered nurse to plan, organize and direct in-home hospice care across home and community settings.

The RN will apply nursing processes, assess patient needs, and collaborate with families to deliver compassionate end‑of‑life care. Current state licensure and CPR are required, with strong communication and ability to work independently within an interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of acute care in an institutional setting and 1–2 years in home hospice care.
• Current licensure in State and CPR certification.
• Bachelor’s degree with hospice care experience preferred.
• Excellent observation, verbal and written communication skills; nursing skills per competency checklist.
• Must be able to lift/transfer patients; valid driver with insured vehicle.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and plans care, revising as patient status changes.
• Re-evaluates patient and family needs; initiates plan of care revisions.
• Delivers preventive and rehabilitative nursing actions; administers medications as prescribed.
• Counsels patient and family; provides health care instructions per plan.
• Assists activities of daily living and supports self-sufficiency and comfort care.
• Communicates with physician regarding patient changes and orders; participates in interdisciplinary meetings.
