Job summary

Lighthouse Hospice in Downers Grove, IL seeks a compassionate Hospice Registered Nurse - PRN to provide patient-centered nursing care in patients' homes or facilities.

You will collaborate with the interdisciplinary team, assess needs, manage symptoms, educate families, and participate in end-of-life planning while maintaining documentation in the EMR.

Requires RN licensure, CPR certification, and a valid driver's license; hospice experience is preferred.

Qualifications

• Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current state RN license and/or as per Nurse Licensure Compact rules; valid CPR.
• Experience: RN in clinical care; hospice experience preferred.
• Skills: Nursing practice, strong communication, compassionate patient-focused care.
• Transportation: Reliable vehicle and auto liability insurance.

Responsibilities

• Provide skilled nursing care, symptom management, medications, and pain relief.

Hospice Registered Nurse - PRN (as needed/pay per hour)

Job Summary:

The Hospice RN is responsible for providing compassionate, patient-centered nursing care to individuals receiving hospice services. This role ensures the highest quality of care by administering nursing care in the patient’s place of residence, and coordinating care with the interdisciplinary team, patients, families, and referring agencies. The RN supports patients and their loved ones through the end-of-life journey.

Job Qualifications:

Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.

Licensure: Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license. Valid and current CPR certification.

Experience: Experience as a registered nurse in a clinical care setting. Hospice experience preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Strong clinical Assessment and critical thinking skills. Excellent communication skills. Ability to work independently and as part of an interdisciplinary team. Compassionate, patient-focused approach to care. Must read, write, and comprehend English.

Transportation: Reliable transportation and valid auto liability insurance.

Environmental And Working Conditions:

Works in patients’ homes and facilities in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather. On- call responsibilities may be required as part of the role.

Essential Functions:

• Provide skilled nursing care, including symptom management, medication administration, and pain relief.
• Conduct comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs.
• Conduct routine and as-needed visits.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Assess caregivers’ ability to meet patient needs and provide necessary education and support.
• Work closely with physicians, social workers, chaplains, hospice aides, and other team members to provide holistic, patient-centered care.
• Communicate changes in patient conditions to the care team and recommend adjustments to the care plan.
• Inform physicians and other personnel of patient needs, interventions, and responses to care.
• Educate patients, families, and caregivers on disease processes, symptom management, nutrition, self-care techniques, and end-of-life care.
• Provide emotional support, guidance, and education to families and caregivers.
• Assist with end-of-life planning and grief support.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Ensure continuity and quality of care through comprehensive clinical and progress notes.
• Follow hospice philosophy, infection control protocols, and regulatory guidelines, including Medicare, Medicaid, HIPAA, and accreditation requirements.
• Participate in quality assessment and performance improvement (QAPI) initiatives.
• Engage in hospice-sponsored training, in-service programs, and professional development.
• Promote the hospice’s philosophy, policies, and best practices for high-quality patient care.
• Demonstrate commitment to professional growth and competency through continuing education.

Requirements:

Physical and Mental Effort:

Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
