Job summary

Swan Hospice in Central New Jersey is seeking an Admissions RN to oversee a primary caseload and provide skilled nursing care to patients in facilities and in home settings. You will assess needs, develop the IDT plan of care, and educate patients and families while working within Medicare conditions.

Collaborate with IDT, supervise aides, and ensure documentation and compliance; NJ RN license required; experience in hospice and home health preferred; driver's license required; full-time role.

Qualifications

• Graduate of an accredited registered nursing program; NJ RN license.
• One year general nursing experience; hospice/home health preferred.
• Valid NJ driver’s license and reliable transportation.
• Ability to work within an interdisciplinary IDT.

Responsibilities

• Provide skilled nursing care to hospice patients per physician plan.
• Perform initial and ongoing nursing assessments and plan of care.
• Teach and mentor patients, families, and staff.
• Collaborate with IDT to coordinate care and ensure documentation.
• Maintain compliance with Medicare Hospice Conditions of Participation.
