Job summary

Elevation Hospice of Colorado is seeking a dedicated Registered Nurse to join our PRN team serving clients from Longmont to Denver. You will provide skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and support families through end-of-life care.

Requirements include an accredited nursing degree, current Colorado RN license, CPR certification, and strong communication and compassionate patient care. Previous hospice or home health experience is preferred.

Qualifications

• Graduate of an accredited nursing program (ADN or BSN).
• Current Colorado RN license and CPR certification.
• Strong communication and patient-family support skills.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Coordinate care with the IDT and referrals.
• Participate in on-call rotations and ensure timely documentation.
• Educate patients and families about disease process and care goals.
• Adapt care plans based on patient needs and acuity.
