Job summary

Affinity Hospice Management LLC in Nashville, TN is seeking a registered nurse to coordinate and deliver hospice care in patients' homes and communities, with emphasis on family education and comfort.

The role requires comprehensive assessments, care planning, medication administration, patient/family education, and collaboration with an interdisciplinary team. Weekend Fri-Mon 5pm-8am shifts are required.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed to practice.
• Minimum 1 year of professional nursing experience in the last 3 years; hospice/home health preferred.
• CPR certification required.
• Valid driver’s license with reliable vehicle.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care; re-evaluates regularly.
• Develops a care plan with goals based on nursing diagnosis and patient/family input.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing needs and provides health instructions.
• Assists patient with activities of daily living and comfort measures.
• Acts as Case Manager for assigned caseload and coordinates care.
• Maintains and submits accurate clinical notes and documentation.
• Communicates with physician and coordinates care with the interdisciplinary team.
• Participates in on-call duties and ensures equipment and services are available.
