Apploi is seeking a Registered Nurse for hospice care with a home health focus in Nashville, TN. The role covers weekends and Fri-Mon night shifts (5pm-8am) as part of a patient-focused interdisciplinary team. You will assess, plan, implement, and document care, while supporting patients and families in home settings.

The ideal candidate holds an accredited RN license, CPR certification, and experience in hospice or home health care, with strong communication and care coordination skills.

Qualifications

• Must be a graduate of an accredited school of nursing with an active RN license.
• Experience in hospice or home health nursing is preferred.
• CPR certification required. A valid driver's license with reliable transportation is needed.

Responsabilités

• Conduct initial and ongoing patient assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Coordinate care with physicians, families, and interdisciplinary team.
• Develop and revise plans of care; administer medications and treatments as prescribed.
• Counsel patients and families on nursing needs and self-care techniques.
• Maintain patient safety and assist with activities of daily living as needed.
• Document observations and care plans; participate in team meetings.
