Job summary

Three Oaks Hospice, Inc. in Saint Joseph, MO is seeking an experienced Registered Nurse to provide skilled nursing care to patients in their homes and coordinate with the interdisciplinary team.

You will assess needs, implement the plan of care, educate patients and families, and participate in on-call coverage. Applicants should have an Associate or Bachelor’s degree in Nursing, an active RN license in Missouri, CPR certification, and strong communication and organizational skills.

Qualifications

• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Current CPR certification.

Responsibilities

• Initial and ongoing comprehensive assessments of patients’ needs and ADLs in the home setting.
• Implement the individualized plan of care and revise as necessary.
• Educate patients and families regarding disease process and care.
• Provide on-going nursing procedures and document progress.
• Coordinate with the interdisciplinary team and referral sources.
• Determine scope and frequency of services based on acuity and needs.
• Perform on-call responsibilities as assigned.
