Job summary

Three Oaks Hospice is seeking a registered nurse to provide skilled nursing care in patients' homes across the South Austin area. You will coordinate care with the interdisciplinary team, assess patients, educate families, and ensure compliant, compassionate end‑of‑life care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office suite experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
• Strong communication and organizational skills and high-standard of ethics and compliance.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's needs and ADLs.
• Implement the individualized plan of care and revisions as necessary.
• Consulting with and educating the patient/family regarding disease process, end-of-life care, nutrition and dietary needs.
• Initiating appropriate preventive and rehabilitative nursing procedures.
• Preparing clinical and progress notes demonstrating progress toward goals.
• Coordinating all patient/family services with the IDT.
• Informing physician and other personnel of changes in patient’s needs.
• Determining scope and frequency of services based on acuity and needs.
• On-call responsibilities and on-call service to patients/families as assigned.
