Job summary

You will perform comprehensive assessments, implement care plans, educate families, and participate in on-call rotations, all while maintaining compassionate patient- and family-centered care.

Qualifications

• Graduate degree in Nursing (ADN/BSN) from an accredited program.
• Current Texas RN license and driver's license.
• Minimum 2 years RN experience; hospice/home health preferred.
• Current CPR certification.

Responsibilities

• Assess patient needs and develop care plan with IDT.
• Coordinate care with interdisciplinary team.
• Provide end-of-life care and family education.
• Perform on-call responsibilities as assigned.
• Document progress notes and communications.
• Maintain HIPAA and ethical standards.
