Job summary

Three Oaks Hospice is seeking a qualified Registered Nurse to provide skilled nursing care to patients in their homes across Texas. You will coordinate with the interdisciplinary team, educate families, and ensure high-quality end-of-life care.

The role requires an Associate or Bachelor's in Nursing, an active RN license, CPR certification, and at least two years of clinical RN experience (hospice/home health preferred). On-call duties may apply.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver's license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.

Responsibilities

• Administer skilled nursing care in patients' homes.
• Coordinate care with the interdisciplinary team and families.
• Prepare clinical and progress notes documenting care goals.
• Initiate preventive and rehabilitative nursing procedures.
• Provide on-call nursing coverage as assigned.
