Job summary

Three Oaks Hospice in Texas is seeking a registered nurse to provide skilled nursing care for patients in their homes and community settings. You will coordinate with an interdisciplinary team, educate patients and families, and document progress to ensure quality end-of-life care.

The role requires an ADN/BSN, an active RN license, CPR certification, and strong communication skills. Hospice/home health experience is preferred. On-call duties are part of the responsibilities.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN.
• Two years’ RN experience; hospice or home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong communication and organizational skills.

Responsibilities

• Provide skilled nursing care in patients' homes or settings per POC.
• Coordinate care with the interdisciplinary team and family.
• Document care progress and outcomes; adjust plan as needed.
• Deliver education to patient/family on disease, care, and comfort.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Participate in on-call coverage as assigned.
