Job summary

Three Oaks Hospice is seeking a Registered Nurse to provide skilled nursing care across patient homes in Texas. You will coordinate care with the interdisciplinary team and families to ensure comprehensive, compassionate support at end of life.

The role requires a valid RN license, CPR certification, and 2+ years of RN experience in clinical settings, with hospice/home health preferred. Strong communication, organizational skills, and computer proficiency are essential.

Qualifications

• :Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience is preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s needs and ADLs.
• Coordinate care with the interdisciplinary team and family.
• Educate patient/family about disease, self-care, and end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes showing progress toward goals.
• Coordinate all patient/family services with IDT.
• Inform physician of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability and plan for ongoing support.
