Job summary

Compassus is seeking a Registered Nurse to plan, organize, and direct hospice care in patients' homes, collaborating with the IDG to tailor Plans of Care. The role emphasizes education, documentation, and coordination for comprehensive at‑home services to patients across age groups.

Required CA RN license and BLS; 1+ year hospice/home health experience preferred. The position includes on‑call duties and a competitive benefits package.

Qualifications

• RN license currently active and in good standing.
• BLS certification within 30 days of hire.
• 1+ year of hospice, palliative care, home health, or skilled nursing experience preferred.

Responsibilities

• Completes comprehensive assessments of patients and families to determine hospice needs.
• Develops, initiates, and revises individualized Plans of Care with the IDG and sets goals.
• Provides professional nursing care using the nursing process and palliative interventions.
• Delivers health care instructions to patients and families per assessment and plan.
• Coordinates with the Patient Care Manager and IDG to ensure continuity of care.
• Completes accurate and timely clinical documentation of patient condition and outcomes.
• Attends IDG meetings to review and update Plans of Care; maintains on‑call duties and productivity.
