Job summary

Anchor Health in San Diego, CA is seeking a compassionate registered nurse to join our hospice team. You will plan, organize and direct home-based hospice care, collaborating with families and the Interdisciplinary team to tailor each patient’s plan.

The role emphasizes patient advocacy, accurate documentation, and coordination with physicians and community providers. A California RN license and CPR certification are required, with experience in home care or hospice preferred.

Qualifications

• Graduate of a NLN-accredited school of nursing.
• Current California RN licensure and CPR certification.
• Minimum of one year of professional nursing experience within the last three years, or a BSN with RN license.
• Two years’ experience in public health, home care, or hospice nursing preferred.
• No management experience required; may supervise hospice aides.

Responsibilities

• Plan, organize and direct hospice care for patients and families using the nursing process.
• Assess and revise patient status and care plans as needed.
• Develop care plans with goals based on nursing diagnosis including family in planning.
• Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
• Complete, maintain and submit accurate clinical notes; document pain/symptom changes.
• Communicate with physicians and community health professionals regarding patient needs.
