Job summary

Enhabit Home Health & Hospice is seeking a licensed RN to coordinate care for patients in the home setting. You will assess patient and family needs, implement interdisciplinary care plans, and ensure all services align with physician orders and applicable regulations.

Ideal candidates have at least 1 year of RN experience, active RN licensure in the practicing state, CPR certification, and strong technologic comfort for mobile devices and documentation.

Qualifications

• Graduate of an approved nursing program and licensed RN in the practicing state.
• Minimum of 1 year RN experience in home health or hospice preferred.
• CPR certification and basic technology skills required.

Responsibilities

• Provide coordination of care and assess patient/family needs to implement the care plan.
• Deliver services per physician orders and the established plan of care.
• Ensure care delivery complies with federal and state regulations.
