Job summary

Compassus is seeking a Hospice Registered Nurse to join our interdisciplinary team and deliver compassionate home care. The RN will perform routine and emergency assessments, adjust plans of care with the IDT, and educate patients and families about symptom control and wound care.

Responsibilities include documenting accurately, coordinating with the Medical Director and attending physicians, and providing after-hours on-call support.

Qualifications

• Strong written and verbal communication.
• Ability to work effectively with the interdisciplinary team.
• Adaptable to crisis situations and remote accessibility requirements.
• Familiarity with care planning and documentation practices.

Responsibilities

• Complete routine and emergency nursing assessments per policy.
• Deliver clinical and psycho-social care per plan of care.
• Document observations, treatments, and changes to the care plan.
• Communicate with IDT, Medical Director, and attending physician.
• Provide education to patient/caregiver on care, disease process, and wound care.
• Maintain bedside documentation and timely IDT narratives and time logs.
• Support home-based hospice care and crisis management to prevent hospitalizations.
