Job summary

BEACON HOME HEALTH in Phoenix seeks a registered nurse to plan, organize and direct hospice care within patients' homes and their communities. The role relies on nursing expertise and community health education to tailor services for individuals and families.

The RN will conduct comprehensive assessments, develop care plans with patient and family input, administer medications, and coordinate care with an interdisciplinary team. On-call duties and ongoing professional development are required.

Qualifications

• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of experience, at least one in public health, home care, or hospice nursing preferred.
• Supervisory experience not required; may supervise hospice aides.

Responsibilities

• Performs initial, comprehensive and ongoing patient/family assessments to determine hospice needs.
• Develops and updates the plan of care with patient/family involvement.
• Administers medications and treatments as prescribed in the plan of care.
• Provides health education and counseling to patient and family.
• Acts as case manager to coordinate care for assigned caseload.
• Completes accurate clinical notes and communicates with physicians and team.
• Participates in on-call duties and supports quality improvement activities.
