Job summary

Qualifications

• Graduate of National League for Nursing accredited school of nursing.
• Minimum of two years of experience, at least one in public health, home care, or hospice nursing preferred.
• Registered nurse with current licensure to practice professional nursing in the state.

Responsibilities

• Performs initial and ongoing patient/family assessments to determine hospice needs.
• Develops and revises the care plan with patient and family involvement.
• Administers medications and treatments as prescribed.
• Coordinates patient care as case manager for assigned caseload.
• Provides health counseling to patients and families and supports daily living activities.
