Job summary

Qualifications

Graduate of National League for Nursing accredited school of nursing.
Minimum of two years of experience, at least one in public health, home care, or hospice nursing is preferred.
Registered nurse with current licensure to practice in the state.
Current CPR certification.
Must be a licensed driver with insured vehicle.

Responsibilities

Performs initial and ongoing assessments of patient and family to determine hospice needs.
Provides professional nursing care using the nursing process.
Develops and initiates a plan of care with patient/family involvement.
Administers medications and treatments as prescribed.
Acts as Case Manager and coordinates patient care for assigned caseload.
Maintains accurate clinical notes and communicates with physician and interdisciplinary team.
