Job summary

Hospice of the West is currently seeking a Full Time On-Call Registered Nurse (RN) to join our compassionate team serving Phoenix and surrounding areas. In this role, you will assess patient needs, coordinate care, and provide emotional support to patients and families during critical moments.

Responsibilities include accurate documentation, responding to urgent and routine requests, and supporting families with education and guidance.

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp a plus.
• Computer proficiency including EMR.
• CPR certification as required by state.

Responsibilities

• Be the Lifeline: Respond to a variety of patient needs, from urgent to routine, your unwavering support will make a difference in critical moments.
• Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
• Maintain Accurate Documentation: Every detail is crucial in delivering high-quality care, both in the patient's home and within our agency and regulatory standards.
• Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones.
