Job summary

VIA Health Partners is seeking a Registered Nurse to deliver nursing services and education in home or facility settings within the North Carolina Board of Nursing scope. You will coordinate plan of care with the interdisciplinary team and serve as the RN member to achieve patient-focused goals.

The role requires 2 years of related experience in hospice/palliative care, active nursing license (RN or LPN), CPR certification, and strong communication skills.

Qualifications

• Diploma or associate degree in nursing required; bachelor’s degree preferred.
• Current active and unrestricted nursing license (RN or LPN) is required.
• CPR required; CHPN preferred.
• Minimum 2 years related nursing experience in life-limiting illnesses.
• Computer proficiency and ability to document accurately.

Responsibilities

• Provide nursing services and education within scope set by NC Board of Nursing.
• Coordinate care plan with interdisciplinary team to meet patient needs.
• Serve as RN member of interdisciplinary group and support care delivery.
• Deliver education and counseling to patients and families about palliative care.
