Qualifications

• Minimum of one-year experience as a professional nurse in an acute care setting.

• Maintain current CPR certification (BCLS).
• Current CPR certification required.
• Must possess a valid Driver's License.

Responsibilities

• Coordinate all Hospice services in assigned counties.
• Provide skilled nursing care according to the physician's plan.
• Supervise Licensed Practical Nurses and paraprofessionals.
• Document all patient interactions in EMR.
• Supervise and evaluate the LPN and CNA care.
