Job summary

Chapters Health System in Florida is seeking an RN for the Hospice House to assess patient and family needs, coordinate care with the IDG, and provide compassionate palliative and supportive care. Strong emphasis on communication and adaptability within a multidisciplinary team.

The role requires an active RN license, hospice or inpatient nursing experience preferred, and ability to travel as needed. EMS/EMR experience is beneficial; flexibility to meet patient needs is essential.

Qualifications

• Current RN license in state providing care or an active compact license.
• Minimum one year inpatient nursing or hospice experience preferred (internal applicants: 6 months LPN in affiliate).
• New grads may be considered in IPU only.
• Experience with EMR/EHR systems preferred.

Responsibilities

• Provide and manage direct care to patients and families as part of IDG.
• Coordinate Plan of Care reflecting patient’s evolving needs.
• Educate patients and families about disease, palliative care, and safety practices.
• Report changes in patient condition to IDG or other health professionals.
• Participate with IDG to evaluate referrals/admissions for level of care.
