Job summary

Chapters Health System in Florida seeks a Per Diem RN to provide compassionate palliative and supportive care within our Hospice House patient/family unit. You will assess needs, coordinate the Plan of Care with the IDG, and communicate with physicians and families to ease patient discomfort.

The role requires an active RN license in Florida, BLS certification, and comfort with EMR/EHR systems. A valid driver's license and reliable transportation are also required for field visits.

Qualifications

• Active RN license in the state of Florida.
• BLS certification from AHA or Red Cross.
• Experience with EMR/EHR systems preferred.
• Must have a valid driver’s license and reliable transportation.
• Strong verbal and written English communication.
• Flexible schedule to meet Hospice House patient needs.
• Hospice or hospital nursing experience preferred.

Responsibilities

• Provide direct care to patients and families as part of IDG.
• Coordinate the Plan of Care with the interdisciplinary team.
• Educate patients and families about Disease process, comfort measures, and safety.
• Report changes in patient condition to IDG and physicians.
• Participate in admissions, assessments, and timely documentation.
• Assist with transportation and discharge planning as needed.
