Job summary

Elevate Hospice is seeking a compassionate Registered Nurse Case Manager to join our mission-driven team providing end-of-life care to patients in their homes across Kansas and Missouri. You will deliver high-quality nursing care, coordinate with the IDT, educate families, and ensure continuity of care while supporting patients and caregivers with empathy.

The role requires an active RN license in the state of employment, at least 1 year of hospice/home health experience, and CPR/BLS

Qualifications

• Must be a licensed RN in the state of employment.
• BSN or CHPN preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Active CPR/BLS certification.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients, coordinating care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medication, and end-of-life care.
• Monitor and update care plans based on ongoing patient assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in interdisciplinary team (IDT) meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
