Job summary

Northern Arizona Hospice, part of the Optum family within UnitedHealth Group, seeks an RN Case Manager to oversee the plan of care for hospice patients and deliver comprehensive nursing care aligned with best practices. You will collaborate with the care team to support physical, psychosocial, and spiritual well-being for patients and families.

This full-time role offers a $7,500 sign-on bonus, flexible scheduling with 3x12 or 4x10 shifts, and a comprehensive benefits package designed to support

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience in a nursing role.
• Current CPR certification and valid driver's license.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care in collaboration with the IDG.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, attending physician, and IDG to provide daily care.
