Job summary

UnitedHealth Group seeks a Registered Nurse Case Manager for Northern Arizona Hospice. You will deliver the Plan of Care, evaluate performance, and collaborate with care teams to support physical, psychosocial, and spiritual well-being for patients and families facing end-of-life challenges.

Offering a $7,500 sign-on bonus for full-time hires and flexible scheduling (3x12 or 4x10) to support work-life balance. Must have RN licensure, CPR, and a valid driver’s license with reliable transportation.

Qualifications

• Active RN license in the state of practice.
• 1+ year of clinical hospice or home health experience.
• CPR certification.
• Reliable vehicle or access to transportation.
• Ability to work independently and in diverse home settings.

Responsibilities

• Perform initial nursing evaluation within 48 hours of referral.
• Assess physical, psychosocial, and environmental needs every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and responses.
• Collaborate with patient/family, physicians, and IDG team daily.
• Instruct patient/family in self-care techniques.
• Maintain accurate clinical notes.
