Job summary

Northern Arizona Hospice, part of LHC Group within the Optum family, seeks a RN Case Manager to oversee Plan of Care for hospice patients, collaborate with the care team, and promote physical, psychosocial, emotional and spiritual well‑being.

A $3,500 sign-on bonus is offered to full-time hires. You will evaluate, plan, and implement care strategies while supporting patients and families in challenging circumstances.

Qualifications

• Current RN licensure in state of practice
• 1+ years of clinical experience
• Current CPR Certification
• Valid driver's license and access to dependable vehicle or public transportation
• Ability to function in diverse home settings

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral
• Identifies patient/family needs and reassesses at least every 15 days
• Initiates and coordinates the plan of care
• Documents problems, goals, interventions, and responses
• Collaborates with patient/family and care team daily
• Instructs patient/family in self-care techniques when appropriate
• Maintains accurate clinical notes
• Recognizes performance and provides development opportunities
