Job summary

Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team in Tinley Park. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey with compassion.

You will collaborate with our Clinical Director, support education for families, and participate in on-call duties as part of a compassionate interdisciplinary team committed to the highest standard of care.

Qualifications

• RN license in good standing.
• At least 1 year of RN experience in medical-surgical/acute care; hospice experience is a plus.
• Competent with computers and EMR software.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct care focused on comfort and quality of life for patients.
• Educate and advocate for patients and families to create personalized care plans.
• Participate in call duties as part of the team.
• Guide patients and families through the hospice journey with compassion.
