Job summary

Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager to join our nursing team in Tinley Park, IL. You will provide hands-on hospice care, coordinate services, and guide patients and families through their hospice journey with compassion and clinical excellence.

You’ll collaborate with the Clinical Director, empower families through education, and participate in on-call duties, ensuring comfort and dignity at every step of care.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice exp a plus.
• Must be computer proficient including EMR experience.
• CPR certification if required by state.
• Current driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Empower Through Engagement: respond to clinical referrals with care and expertise.
• Collaborative Care Champion: partner with Clinical Director to assess eligibility for hospice services.
• Nurturing Direct Care: deliver hands-on hospice nursing care and coordinate services.
• Nursing with Heart: provide care rooted in nursing principles to ease pain and support comfort.
• Educator and Advocate: educate patients and families to create personalized care plans.
• Call of Compassion: participate in on-call duties as part of the team.
