Job summary

Sanctuary Hospice is seeking a compassionate RN Case Manager to work per diem across the Tinley Park area. You will deliver direct hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey alongside a dedicated interdisciplinary team.

You will collaborate with the Clinical Director, maintain high clinical standards, and participate in ongoing onboarding and professional growth with our comprehensive benefits package.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice exp preferred.
• Must be computer proficient with EMR systems.
• Current CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Empower Through Engagement: respond to clinical referrals with care and expertise.
• Collaborative Care Champion: work with Clinical Director to assess eligibility for hospice services.
• Nurturing Direct Care: deliver hands-on hospice nursing care and coordination.
• Nursing with Heart: provide comfort and quality of life through proven nursing practices.
• Educator and Advocate: educate patients/families to create personalized care plans.
• Call of Compassion: participate in on-call duties as part of the team.
