Job summary
Crescent Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager to join our dedicated nursing team serving the Augusta community. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of the hospice journey.
As a valued member of our interdisciplinary care team, you’ll work with compassionate professionals to ensure The Best Care Possible—care grounded in compassion, clinical excellence, and connection.
Qualifications
- Current unencumbered RN license.
- Minimum 1 year RN experience in med-surg/acute care; hospice exp a plus.
- Proficient computer skills, typing, and EMR experience.
- CPR certification as required by state; valid driver's license and reliable transportation.
Responsibilities
- Respond to referrals with compassionate, informed care.
- Assess client eligibility with Clinical Director for hospice services.
- Coordinate direct patient care to maximize comfort and quality of life.
- Deliver nursing care rooted in proven principles and compassion.
- Educate and empower patients and families to participate in care plans.
- Share call duties to ensure care availability when needed.
