Job summary

Crescent Hospice in Greenville is seeking a compassionate Per Diem Hospice RN Case Manager to join our dedicated nursing team delivering end-of-life care across the community. You will provide hands-on care, coordinate services, and guide patients and families through every step of their hospice journey.

Schedule: Per Diem, Monday–Friday, 8 am to 4:30 pm, with territory coverage in Greenville. You’ll work as part of an interdisciplinary team, grounded in compassion, clinical excellence, and

Qualifications

• Must hold an active RN license and be CPR-certified where required.
• Hospice experience is a plus.
• Strong communication and educational skills with patients and families.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with the Clinical Director and care team to assess patient needs.
• Support patients and families with education and care planning.
