Job summary

Elara Caring is seeking a Registered Nurse Case Manager to advance hospice/home health care. You will assess patient and family needs, monitor conditions, and coordinate care with physicians and the care team to ensure timely, compassionate support across care settings.

The role requires an active RN license, 1+ year nursing experience, valid driver’s license, and ability to travel between patient homes. We offer comprehensive benefits and a strong supportive team.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Ability to frequently lift up to 50 pounds and assist patient transfers.
• Valid driver's license and ability to travel between patient homes.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
