Job summary

Elara Caring in Ohio is seeking a Registered Nurse Case Manager to deliver compassionate hospice care to patients and families. You will assess needs, coordinate interdisciplinary plans, and educate caregivers in settings ranging from homes to facilities.

The role requires a current RN license, a nursing degree, and at least one year of clinical nursing experience. Travel between patient homes is expected; reliable transportation and a valid driver's license are required.

Qualifications

  • Current State license as a Registered Nurse.
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.

Responsibilities

  • Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
  • Monitor patient condition and communicate significant changes to the physician and care team.
  • Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
  • Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
  • Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
