Job summary

Elara Caring is hiring a Registered Nurse Case Manager to join our team, focusing on delivering compassionate hospice and home health care. The role requires assessment, plan-of-care development, and collaboration with the interdisciplinary team in patient–centered care.

Qualifications

• Graduate of an accredited Nursing program.
• Current State License as an RN.
• 1 year of experience in a clinical care setting.
• Experience in hospice or home health environment preferred.
• 50% travel required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess hospice care patients and families to identify needs and document findings.
• Coordinate and communicate the Interdisciplinary Plan of Care with the team.
• Provide patient/family education and bereavement support as appropriate.
• Report significant changes in health condition to Clinical Supervisor and team.
