Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to join our team and deliver compassionate care across patient homes and care settings. The role focuses on comprehensive patient assessments, timely communication with the care team, and education of patients, families, and staff.

The ideal candidate will have a valid RN license, a nursing degree, and at least one year of clinical nursing experience, with hospice or home health experience preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
