Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our team and drive compassionate home-based care. You will assess patients and families, coordinate interdisciplinary care, and educate staff across settings while upholding our mission and care standards.

You will report to the Clinical Team Manager and travel up to 50% to patient locations, facilities, and partner sites. A nursing license is required and hospice or home health experience is preferred.

Qualifications

• Must be a graduate of an accredited nursing program.
• Current RN license in the state.
• Minimum 1 year of clinical care experience.
• Hospice or home health experience preferred.
• 50% travel required.
• Valid driver’s license and reliable transportation.
• You will report to the Clinical Team Manager.

Responsibilities

• Ensures that all activities align with the vision of Elara Caring’s board, executive team, and Hospice leadership.
• Assesses hospice care patients and families to identify physical, psychosocial, and environmental needs with documentation and evaluations.
• Assesses patient needs across physical, psychological, social, and spiritual factors.
• Communicates significant findings to the Clinical Supervisor, physician, facility, and other involved personnel.
• Counsels, instructs, and includes patient and family in the Interdisciplinary Plan of Care.
• T eaches hospice philosophy and pain management to staff in alternate care settings.
• Provides support at time of death and performs bereavement assessment.
