Job summary

Elara Caring seeks a compassionate Hospice RN Case Manager in Dallas, TX to assess patients and families, monitor conditions, and coordinate the interdisciplinary plan of care. You will educate staff, support caregivers, and oversee pain and symptom management across hospital, home health, and nursing home settings.

The role requires an active RN license, 1+ year of clinical nursing experience, and the ability to lift up to 50 pounds.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate/Diploma/Associate/Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license and reliable transportation for travel between patient homes.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate changes to physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families.
