Job summary

Luminary Hospice in Cincinnati, OH seeks a Registered Nurse Case Manager to plan, direct, and oversee hospice care for patients in their homes and communities. You’ll build plans with a focus on comfort, dignity, and family involvement, coordinating care through an interdisciplinary team.

Responsibilities include comprehensive assessments, developing and updating care plans, administering prescribed therapies, and guiding families on self-care. Strong nursing skills and compassion are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in State and CPR certification.
• Bachelor’s degree, with hospice care experience preferred.
• Management experience is not required.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Completes an initial, comprehensive assessment of the patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Assesses patient status and writes plan of care with regular re-evaluation.
• Initiates and revises plan of care as patient needs change.
• Delivers nursing treatment and administers medications as prescribed.
• Counsels patient and family and provides health instructions as appropriate.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Completes notes and communicates with physician regarding changes in condition.
• Maintains safe environment and supports continuity of care with team.
